My Therapist Group is a sole-practitioner physiotherapy clinic operated by Ash Samarakoon at 35 Partridge Crescent, Frankston VIC 3199. This policy explains how we collect, hold, use and disclose personal information and health information when you visit our website, contact the clinic, make a booking or receive physiotherapy care.
INFORMATION WE COLLECT
We may collect identity and contact information such as your name, date of birth, address, telephone number and email address.
We may collect appointment and administrative information such as booking details, referral information, fees, invoices, and Medicare, Enhanced Primary Care or private-health details where relevant.
We may collect health information needed to assess and provide care, including symptoms, medical and treatment history, assessment findings, clinical notes, referrals, reports, scans and consent information.
When you use our website, limited technical information may be collected, including your IP address, browser or device information, pages visited and website interactions recorded through Google Analytics 4.
HOW WE COLLECT INFORMATION
We generally collect information directly from you during appointments, by telephone or email, through online bookings and forms, or when you use our website. With your consent or where otherwise authorised by law, we may also receive relevant information from a referring practitioner, another healthcare provider, an insurer or another person involved in your care.
Online appointments are managed through Acuity Scheduling on behalf of My Therapist Group.
HOW WE USE AND DISCLOSE INFORMATION
We use personal and health information to:
• arrange and provide physiotherapy care;
• communicate with you about appointments and treatment;
• maintain clinical and administrative records;
• manage fees, billing and referrals;
• support clinical quality, safety and continuity of care; and
• meet legal, professional, insurance and regulatory obligations.
Where reasonably necessary, information may be disclosed to healthcare providers involved in your care with your consent or other lawful authority; service providers that support the clinic, including Squarespace, Acuity Scheduling and Google; or insurers, professional advisers, regulators and authorities where required or permitted by law.
We do not sell personal information. We do not use health information for targeted advertising.
ONLINE SERVICES AND OVERSEAS PROCESSING
Squarespace hosts this website, Acuity Scheduling manages online bookings, and Google Analytics 4 helps us understand aggregate website use and booking, telephone and directions interactions. These providers may process or store information outside Australia, including in the United States and other locations described in their own privacy policies.
SECURITY AND RETENTION
We take reasonable administrative, technical and physical steps to protect personal and health information from misuse, interference, loss, unauthorised access, modification or disclosure. No internet or electronic storage system can be guaranteed to be completely secure.
Information is retained for the periods required by applicable law and professional standards. When it is no longer required and lawful to do so, it is securely destroyed or de-identified.
ACCESS AND CORRECTION
You may request access to personal information we hold about you or ask us to correct information that is inaccurate, out of date, incomplete or misleading. We may need to verify your identity before responding and will handle requests in accordance with applicable law.
PRIVACY COMPLAINTS
If you have a privacy concern or complaint, please contact us using the details below. We will review the matter and respond within a reasonable time. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner at oaic.gov.au or the Health Complaints Commissioner Victoria at hcc.vic.gov.au.
