What your sciatica assessment may include

Sciatica is commonly used to describe symptoms that travel along the leg, but not every episode of buttock or leg pain has the same source. Symptoms may relate to irritation or sensitivity of a nerve in the lower back, while hip, joint or muscle problems can sometimes produce a similar pattern.

Ash will discuss where the symptoms travel, how they began, what changes them and whether you have numbness, tingling or weakness. Assessment may include lower-back and hip movement, walking, strength, sensation, reflexes, nerve sensitivity and tasks relevant to work, daily life, gym or sport.

Common questions about sciatica

When should I see a physiotherapist?

Consider an assessment when back or leg symptoms are limiting sitting, sleep, walking, work, exercise or daily activity, when they are not settling, or when you are unsure what movement is appropriate.

Does sciatica always mean a slipped disc?

No. Disc-related changes are one possible contributor, but symptoms and scan findings do not always match. Assessment considers the full clinical pattern rather than assuming one cause.

Do I need a scan before physiotherapy?

Often not. Ash will assess the symptoms and explain whether medical review or imaging may add useful information.

When do sciatica symptoms need urgent medical care?

Seek urgent medical assessment for new loss of bladder or bowel control, numbness around the groin or saddle area, rapidly worsening leg weakness, or severe symptoms affecting both legs. These features can indicate significant nerve compression.

Treatment and return to movement

Your plan is tailored to the findings, irritability of the symptoms and the activities that matter to you. It may include explanation and reassurance, changes to aggravating positions or loads, graded movement, lower-back and hip exercises, strength work and a progressive return to sitting, work, walking, gym or sport. Hands-on physiotherapy may be used where appropriate as part of the wider plan.
